Bringing Living Back Into Our Patients' Lives

Our sole focus at CHHA is improving the quality of life for our community members who need our services. We serve our community members who are homebound due to illness or are experiencing a change in their health status.

Our professional staff provides care in the safe and familiar environment of our patients' homes. They are trained to identify our patients' health and functional issues, safety concerns and equipment needs to facilitate optimal recovery in managing safe functional mobility of Activities of Daily Living (ADLs).

Crescent Home Health Agency staff maintains a close working relationship with referring physicians, Durable Medical Equipment vendors, clinical laboratories and pharmacies to ensure safe management of patient needs.
