Our Home Healthcare services are carefully customized for your needs.

Crescent’s professional and compassionate clinicians specialize in chronic disease management, wound care, IV and infusion therapies and post-surgical rehabilitation. We will collaborate with patients, caregivers and family to develop an individualized care plan for optimal outcomes.

Skilled Nursing

Our skilled nursing staff is licensed, experienced and knowledgeable in disease management. Our goal is to manage patients effectively in the comfort of their home to minimize re-hospitalization and decrease costs.

Rehabilitation

Our Physical Therapists treat patients in the comfort of their home for unsteady gait, dizziness and balance disorder to reduce falls risk, muscle weakness and lack of endurance/functional capacity, unsteady gait and diminished ambulatory skills.

Home Care Services

We also provide Home Care services, which entail “non-clinical” or “non-skilled” care by professional caregivers. Our Home Care staff assists with everyday tasks like bathing, dressing, grooming, cooking, cleaning and transportation. This differs from our Home Healthcare services, which are more medical in nature and provided by skilled nurses and therapists.

Crescent Home Health Agency is a Community Health Accreditation Program (CHAP) skilled Home Healthcare provider for Franklin, Jefferson, St. Charles, St. Louis City and St. Louis Counties in Missouri and Clinton, Jersey, Madison, Monroe, St. Clair, and Washington Counties in Illinois.
