A case manager, under the supervision of the Clinical Director, will assess and develop an individualized care plan to address patient specific needs. We involve patient, caregiver and family in care planning decisions, patient specific goals and education and training. Crescent Home Health Agency orientates and trains a clinically diverse staff for efficient and effective skilled services and optimal outcomes. The following services are provided to communities that we serve.
Our skilled nursing staff is licensed, experienced and knowledgeable in disease management. Our goal is to manage patients effectively in the comfort of their home to minimize re-hospitalization and decrease costs.

Home Health Aide (HHA)
We provide this service to patients who are unsafe and unable to groom or bathe themselves due to severe weakness and or assistance from caregiver or family. Our skilled nursing staff makes the determination upon a thorough initial assessment of the patient for HHA needs. An aide care plan for the patient is developed and supervised on a regular basis by the clinical staff for appropriateness of services.
Our physical therapists treat patients in the comfort of their home for unsteady gait, dizziness and balance disorder, muscle weakness, lack of endurance/functional capacity and diminished ambulatory skills. Our therapists work with patients on fall prevention and balance/coordination training, recovering and restoring functional independence, improving strength and endurance, retraining in safe functional mobility and safe ambulatory skills on levels, stairs/steps and uneven surface management with or without assistive devices and designing individualized Home Exercise Programs (HEP) for follow up after discharge from services independently or with the help of a caregiver.
Our occupational therapists will develop a care plan that will incorporate the techniques necessary for safe and independent self care and cognitive training. Patients are trained on safe grooming, dressing techniques for upper and lower body, transfers in shower/tub, bathing, toileting and use of adaptive devices for safe management of activities of daily living (ADLs).
Our speech therapists serve individuals with profound speech and language issues, memory issues and swallowing difficulties due to a variety of neurological or physiological reasons. They use expertise and knowledge of augmentative communication devices and language stimulation and assist in improving swallowing disorders and communicative activities for daily living.
Our Licensed Medical Social Worker (MSW) evaluates our patients' individual living needs and support systems for safe living in their own home or living in community based programs. The MSW will provide patient counseling and search and explain benefits available through other organizations and community resources.
